Prepare for Your Visit
1. Take note of how much medication you have left. If you will need more soon, please tell your nurse at the beginning of your visit.
2. Fill out the Reason for Your Visit form, print it, and take it along with you. This will help Dr. Grimshaw to help you!
3. Print the Questions to ask Dr. Grimshaw form now so you can have your own copy of what happened during the visit and what you should do after the visit. Use this form like a string around your finger, it will help you to remember the important details!
